Online Forms

Most everything at Dutchess is done with paper forms, this is mildly wasteful and inefficient. Since working in the Office of Community Relations it seems to have become my job to convert some of these forms to be submitted online instead. Good news for everyone!

Technical Info

The scripting is currently just by-hand brain-dead piecing together of old projects, namely a script started eons ago by these guys, which I got to swoop in an fix up for my own purposes. Eventually as these things evolve I hope to make a centralized system for creating and using these that will be tied to a single very useful database and live behind an LDAP login so the mailroom doesn't get duplication service requests from people off the street.

Currently

I am looking for software packages that would facilitate this. So far I have found mostly online monthly services, some of which look great, but are not what we are after. The only two I have been able to find that seem to be of value are Form Maker Pro and PHPForms. Between the two of them I am a fan of Form Maker Pro in lieu of writing these by hand. I will write a review of it if we end up using it.
